• [ ] Mode (A / B / C) stated; Mode B routed to /document-existing; Mode C has confirmed baseline (inventory hoặc FS as-built).
• [ ] ba/system-profile.md read (§1–§4) và không còn là template trống.
• [ ] Source input listed.
• [ ] project-context.md checked.
• [ ] project-decisions.md checked.
• [ ] Existing docs/working artifacts checked.
• [ ] Related KB checked when relevant.
• [ ] Correct skill/checklist/template selected for current step.

Output Quality

• [ ] Output saved to mapped folder.
• [ ] File naming follows version/date rule.
• [ ] Source input stated in artifact.
• [ ] Fact, Assumption, Open Question, Recommendation separated.
• [ ] No unconfirmed assumption is treated as fact.
• [ ] No decision is written as final unless BA confirmed.
• [ ] Scope/rule/data/API/user flow gaps become open questions.
• [ ] Downstream traceability is clear.

Gate Control

• [ ] Current step/gate explicitly stated.
• [ ] Artifact self-reviewed before reporting back.
• [ ] BA is asked for Approved, Approved with Notes, or Need Revision.
• [ ] Workflow does not move forward without clear BA response.
• [ ] If Need Revision, current artifact is revised before next step.

Knowledge Base

• [ ] KB update is only proposed before Gate 6.
• [ ] Shared KB is updated only after BA approves KB proposal.
• [ ] Reusable Knowledge and Project-Specific Knowledge are separated.
• [ ] 00-index.md is updated when shared KB changes.
